[Plugin: Fast Secure Contact Form] Label being rendered as a required checkbox

I just installed this plugin and I think it may be awesome and plan to donate.
In form 1 when I set up a yes or no check box a third check box shows up using the label attribute. How do I stop this from rendering.https://www.ads-software.com/extend/plugins/si-contact-form/
