[Plugin: Fast Secure Contact Form] Redirect problem to php script

I am using the latest version of SI Contact Form on the following pages
1. https://pezenaspropertyinvestments.com/contact/
2. https://pezenaspropertyinvestments.com/investing-with-ppi/complimentary-report/
3. https://pezenaspropertyinvestments.com/investing-with-ppi/register-with-ppi/

#1 works fine with the plugin doing a simple redirect to the homepage

#2 was working previously but now presents a 404 page when trying to redirect to the following URL

#3 is a new form operates the same way as #2 (to another cloned script) and gets the same result.

The 404 page for #2 has a title tag as follows:

I have written to the hosting company (namesco) but if anyone is able to offer any advice, it would be much appreciated.
