As far as the layout, yes, that example has a similar booking flow to Simply Schedule Appointments.
Your users should be able to pick a service -> pick a date -> pick a time -> fill out form and submit.

No, I’m afraid we don’t have a way to add images to the appointment type listings. This is actually the first request we’ve had for something like this, but I can add it to our to-do list for future consideration.
