Plugin for quiz?

    I am absolutely new to WordPress and I need your help. I’m looking for a plug-in that can help me establish a quiz and generate a report from the results of the quiz.

    My work wants to set up a WordPress site that can enable people to take a quiz and then produce a personalised report for them at the end of the quiz.

    The quiz will be split into 5 sections with 5 questions in each section (total of 25 questions). Each section’s answers will be used to generate a different paragraph in the final report. With each section there will be three possible outcome weightings (weak, medium, strong) and each weighting will have a pre-written block of text.

    We would also like to gather some information about he people taking the quiz before they start the quiz; such as ‘where are you from?’.

    We’d like to be able to store the outcomes of these questions as aggregate data as well.

    Hope that makes sense. Any help or pointers in the right direction would be much appreciated.
