plugin for survey with points attributed to each question

Hi

I am looking for a survey plugin.
I need a survey of 20 questions, and each questoin has 6 possible answers with a particular number of accumluated points for each answer :
example :
question 1: Are you feeling good ?
answers :
1. strongly disagree (if the user check this answer he’s got 0 points)
2. disagree (if the user check this answer he’s got 1 points)
3. slightly disagree (if the user check this answer he’s got 2 points)
4. slightly agree (if the user check this answer he’s got 3 points)
5. agree (if the user check this answer he’s got 4 points)
6. stronly agree (if the user check this answer he’s got 5 points)

I have tried different plugins but none can do that (only wp simple survey but it’s only free for one survey).
