[Plugin: Front File Manager] Form wasn't displaying properly-how I fixed it.

I downloaded this plugin and it worked great, but for some reason the text fields at the bottom of the form were not displaying properly. I don’t know why. It may have something to do with my theme (Graphene) or something else. Anyway, It was displaying the text fields really weird, so I had to change the code it used to display the category select, email, and message part of the form. I am posting this so that if anyone else has this problem they can fix it easily.

Here is the code that you will need to edit. Open front-file-manager/front-file-manager.php in the plugin editor, and scroll down to this code:

There may be a better way to do it, but this is the only thing that I could figure out. If anyone else is having this problem, then here is the solution for you.
