Plugin – Help – Multiple Pages

I have this code in my project, and what i want to do, is when I use the submit button, that it runs a script to create a database, my problem is… how can I make it run the script? what do I ahve to put in the “Action”? And after the script creates the database, I want him to return to this page…

Execute a script;
