[Plugin: Inline Upload] Custom Fields for User Input

Is there a way to enter some custom fields for the user to enter information (mandatory fields as bonus) which would be included with the inline upload.
A usage example would be, user has to include a name.
That %name% variable is sent in the notification email. Maybe even included in the filename..(I know that the wordpress username can be included, but this example would be for uploadrole=”all”.)
This functionality has been asked already but that thread has been closed with no answer.
