[Plugin: Job Manager] Application page is blank

Using 0.7.18, did an install from scratch. Changed the page URL, and added two jobs, one real and one test. They show up as expected in the jobs listing page, but clicking the APPLY button for the real job takes me to a page with only a title and no application fields. The test job looks correct.
I checked the application settings, and there are fields set to show for all the categories. I’ve also deleted the real job and re-added it, but the application still does not appear.
Also, I noticed that the apply link is going to the default URL (/jobs-2) instead of what I changed it to.
