[Plugin: MailChimp Widget] Add to theme , in page not sidebar

Hi

I am looking for a mailchimp plugin which support multiple lists – like this one does! and that allows me to add the forms to pages and not just in the sidebar widgets. Is there a short code or seomthing i can use to load the lists into the different pages?
