[plugin: need user input to link to predfined URL's]

Hi i need to find a plugin that can take a user input (4-digit) code and take them to a URL coinciding with the specific code (predefined list created by me). right now i just have all the codes listed out as links https://www.yevohawaii.com/joint-the-hawaii-team …any help would be much appreciated!
