Plugin needed for client file uploads

It’s a plugin for contact forms, but it could work for what you need. Easy for the end users to attach / upload files, add note(s) with the files (since it’s a form they are filling out) and upload. On the admin end, I think it keeps track of the uploads in a database of sorts, and is viewable from the WP Admin pages. Also, it will send you an email notification when someone uploads a file.

I’ve personally never used it like this, but I thought it might be a good start.
