[Plugin: Networks for WordPress] Unable to Add Sites to new network

Sequence of events .. fresh WP install, enabled multisite, added Networks for WordPress pluging. Multisite configured for sub folders.
Two days ago, I test the following.
1. Created a new network at a TLD. Worked fine
2. Created a new network at a subdomain of the TLD. Worked fine
3. Created a site on the network in #2. Worked fine.Fast forward to one day ago.
I attempt to repeat the above with a fresh TLD .. some DNS issues with propagation from our registrar .. things fail and behave very strangely. I chalk it up to trying to do things when DNS is flaky and walk away.
Today, I delete the entire thing and start over with same basic installation as outlined above.
1. I go directly to creating a new network on a subdomain of a TLD. Seems to work fine, although today I note that the navigation in the dashboard gets very odd. Once in the dashboard of the new network, the only path back to the master network admin is to first bring up dashboard for the main site.
2. Larger problem though … (I do have DOMIAN_CURRENT_SITE commented out in wp-config) .. I don’t have an option to create new sites on my new network.
Does this ring a bell for anyone?
