[Plugin: Newsletter Sign-Up] Configuring the Aweber

I am struggling with integration part of Aweber. I have adding the required data as desired in the plugin setting page. I have added the required setting, but it throws an error,

“An Error Occurred
The form you used to subscribe is not set up properly.

Please push the “Back” button and notify the website owner.”

Please can you highlight the steps required and what is required in which field??
