[Plugin: NextGen Gallery] Album Thumbnails not showing

I’ve tried everything I can think of but cannot get album thumbnails to show. I did verify each album has a default/cover image set and I’ve disabled all other plugins. This was working at one point but now it shows a white box with no thumbnail image.

Any ideas?
