Plugin not activate

    1. When did you download this plugin?
    2. What version of WordPress are you using?
    3. Can you send us the URL and WP admin credentials of the blog where you have installed the plugin.

    The plugin has been successfully tested with WP 3.4.2 and WP 3.5.

    Sir I have same as above my plugin get crash after uploading on live server..please help thanks..
