Plugin Not Working (#200 authorization error)

When I try to use the plugin I get the following message: Add Link to Facebook – Motivation: Add link: Facebook error: (#200) The user hasn’t authorized the application to perform this action @ 2014-05-01T15:16:37+00:00
I have followed the FAQ extremely carefully, I have reactivated the plugin several times and even created a second app just to test if the permissions were all being correctly set.
I also tried disabling plugins…. any other suggestions? :/
Google search + FAQ did not help, my theory is that for some reason the app cannot properly grab all of my facebook permissions and there is no clear way to add them.
