Plugin not working correctly / breaks sidebar

Shame I didn’t get any replies on my previous topic (https://www.ads-software.com/support/topic/star-rating-rich_reviews_form-doesnt-work) but the latest version 1.6.3 things got really messy.
The star option (only one I am interested in) still does not work. Did a clean instal after some translation modifications but still nothing. With the last update my sidebar got pushed down, probably because of the malfunctions in the star rating and the “send” button which was also broken. Installed version 1.6.2 which does not do this to my theme and where the send button works. The star rating however is still not working. Anyone has any idea’s where to start looking?
