Plugin not working: “no shipping methods available” on cart

Hi,
I’ve followed the instructions and tried to deactivate / activate the plugin, but to no avail. The shipping debug mode shows I’m being matched for one of the shipping zones where flexible shipping has been set up, but it seems the plugin is not being invoked.
Where should I send the status report to so you can have a look, and what other information should I supply you with? I can make a login for you on the website in question so you can take a look of course.
