For the base PMPro membership level, we’d be able to give you some sample code based on a few simple fields you want to add. If you have something more complicated (more than say 4 fields or some logic within the fields, etc), you’ll want to sign up for our “Do it for me” plan.

i just need to add 6 text fileds.only the name different.
