[Plugin: PHPBB Single Sign On] captcha and resending passwords in phpBB broaken

If a user request a new password the new one doesn’t work and they still have to use the old one (and they forgot it).
If a user tries to loin more then the max allowed they should be presented with a captcha challenge but aren’t.
