[Plugin: PHPList] Adding additional fields

I’m trying to use the PHP List plugin by Jesse Heap and my problem is I want to add additional fields beyond the default two and I followed the info from this tutorial (https://projects.jesseheap.com/adding-additional-custom-fields-to-wordpressphplist-plugin-2006-12-09.htm#Demo) but it’s not working for me.
