[Plugin: Post Avatar] Plugin Causes Upload/Insert Media Button to Fail

I am able to insert media from both the Edit Post and Add New Post screens.

Are there any other plugins that you are using?

My WordPress is current (3.3.1) and my PHP is semi-current (PHP 5.3.3 – built: Feb 2 2012 23:47:49). All of the plugins listed below are current as per WordPress. I have disabled each plugin individually, and the only plugin which causes the issue is Post Avatar. With Post Avatar enabled, clicking on the “Upload/Insert” button in Edit Post or Add New Post fails to do anything. Disabling Post Avatar allows the button to function again.

It’s mostly likely a javascript issue though the javascript that I use for Post Avatar has nothing to do with the Media Uploader.

I have made some changes as to how the javascript is enqueued.

Removed plugin, downloaded plugin from link, extracted, chmod/chown, activated in WordPress, Upload/Insert button does nothing. Disable plugin, Upload/Insert functions again.
