[Plugin: Quick Post Widget] Only one category to chose from

I have qpw installed on a page.

I have 5 categories.

Regardless of category setting (include all / exclude all etc) and admin/user roles, I am only able to see/selct one category. This is regardsless of checkbox or dropdown. It is also regardless of any default category chosen at the widgit level.

The category I see is the first I created.

I have other plugsin installed:
UAM
subscribe2
contact form 7
new user approve
toolbar removea
wp user control

tl;dr: qpw sees my categories in settings tab, but regardless of settings, only show one (the same) category on the front end.
