[Plugin: RSVPmaker] Yes/No and Radio Buttons do not align

I am using the RSVP form within a wordpress template, and I cannot get Yes and No to line up with their radio buttons. Where can this be adjusted?
Also, is it possible to add more response options to the list besides Yes and No?
