    Not necessary the REQUEST_URI or PHP_SELF. Remove the instructions <?php echo $_SERVER['PHP_SELF'] ?> and try again. Example, replace:

    <?php echo $_SERVER['PHP_SELF'] ?>?page=sidebarTabs.php&action=edit&new=1&...

    by

    ?page=sidebarTabs.php&action=edit&new=1&...
