[Plugin: SimpleMap] need a search form on my homepage

i added the [simplemap] on my locations page and it works good. the problem now is that i wanna add the search form on my index page without the map so what i did was to copy the form on the locations page and paste it on my index page when i search for a zipcode it redirects to the locations page but not display any results anyone can help me?

my main function is to have the map on my index page and on the locations page and when users search for a zipcode using the search form on the index page they will get redirected to the locations page and the results will be displayed on the locations page is this possible?
