[Plugin: Social Sharing Toolkit] Follow on Twitter label hard coded with MarijnRongen

Hi,
just to inform that on the follow widget, the label “Follow @marijnrongen” is hardcoded whatever the twitter account is.
It neds just a fix in function get_follow_twitter.
