[Plugin: Subscribe To Comments Reloaded] Problem with alignment

The subscribe to comments reloaded checkbox and text are showing up in a column instead of spread out across the page. I turned off “enable default checkbox” and placed the code as instructed where I wanted the form to show up. This made it show up correctly but it didn’t subscribe my test person even when checked. So, for now it is back in the column format, and it works. Any ideas?
