[Plugin: SurveyMe] Admin's name and email show up in built-in mandatory name & email fields

Great plugin! I just set up several different surveys, and really liked how easy the plugin is, but noticed that when I viewed the page, the mandatory built-in Name and Email fields were pre-filled with the WordPress admin’s name and email.

Obviously, we’d like that to be the name and email of the person filling it out – how do we remove that?
