[Plugin: SurveyMe] survey wont submit

i just finished making the survey for my site, but upon testing it doesn’t submit the survey when i press submit…it simply refreshes the page with all of my answers still intact.
the ‘thank you’ screen that should appear after the survey is not showing up.
please help!
