[Plugin: TDO Mini Forms] No Form 2 in sidebar widgets?

I’m loving TDO Mini Forms, because it lets my guests create posts.
I created two forms of similar formats, and differ only in the size of the text fields. I built Form 1 for a page, while Form 2 is for the sidebar.
Unfortunately, when I looked into the Widgets tab for my WordPress admin page, I couldn’t find “Form 2” on the list of available widgets. All I can see are TDOMF Admin Info, TDOMF Form 1, TDOMF Recent Submissions, and TDOMF Top Submitters.
