[Plugin: TDO Mini Forms] WordPress MU Default Author

In WordPress MU Version 3.0, I cant set the Default Author. The dropdown list is empty and when I manually add a subscriber as a user the list is still remains blank.
When I click the “Create a dummy user ?” link a blank wordpress admin page is displayed.
I found the following forum posts with a similar issue.
