Plugin to auto-generate url after inputting a certain ID

Ok, We have customers that will have specific ID’s and they will need to go to a website to fill out a form and the url of this site must have there specific ID attached to the end of the url.

IS there something that customers can input there ID into a text box, and then the plugin will automatically attach this user id to the end of the pre-defined url and then redirect them to that url with the ID attached to the end?

Anyone heard of such a thing?
