Plugin Update

Hi.
Do you still support this plugin?

I get a blank screen when parsing a test form:
a sample of the code used:

<tr><td>pl01</td><td>silver plate</td><td>5.88</td></tr>
<tr><td>cp01</td><td>silver cup</td><td>3.99</td></tr>
<tr><td>dh02</td><td>Grey dish</td><td>9.99</td></tr>
</tbody></table>

I am prompted to give the new form a title, but it simply creates a new blank form.
Any help would be greatly appreciated, I have a form with over 500 fields to import and im looking for a simpler solution to import them rather than manually create them.
