[Plugin: User Photo] Add a header on the admin page

Nice plugin, works like a charm thus far.
I would encourage you to add the following line:
<h3><?php _e('User Photo', 'user-photo'); ?></h3>At 586 and 588 – this way your table will be titled. Otherwise it gets a bit lost on the user profile page.
