[Plugin: User Submitted Posts] Multiple uploads

i put this on my site for a photo contest. user submit an image and a short story( post content) all posts are making it in but only about 1 out of 3 images are making it in. i have a huge disclaimer asking users to size their images down to under 1 mb. still only 1 out of 3. Any ideas?
