[Plugin: Viper's Video Quicktags] Buttons no longer showing up in post menu

I have been trying to find an answer to this for the past 30 minutes, but unfortunately I have come up empty and need to get back to work.
I have checked this in Firefox 3.6.8 and Safari 5.0, and both have the same problem with the viper buttons missing from post menu bar, however they are visible in the HTML bar. See screenshots below.
