[Plugin: W3 Total Cache] Lost My Menu Bar

BY process of elimination I think this is being caused somehow by caching…
I can log in to WP but then on subsequent pages the black menu bar at the top of the screen is no longer there so I constantly have to visit the admin page over and over. What could be causing this?
