    “Host minified CSS and JS files” is selected and the CDN seems to be working fine (Rackspace Cloud Files), but the minified JS and CSS are being served from the main site domain instead of the CDN.

    I’ve cleared all w3tc caches, cleared browser cache, uninstalled w3tc, deleted w3tc-related files & dirs, removed w3tc code from .htaccess and re-installed w3tc.

    All transfers to CDN look okay.

    Minified files were being called from CDN prior to changing my “Site Address” as suggested here: https://codex.www.ads-software.com/Giving_WordPress_Its_Own_Directory

    Changing my “Site Address” back to the original setting (and deleting all w3tc caches and files) did not help.

    Does anybody have any hints to help me get to the bottom of this?

    I think I have solved my issue with the following:

    1.) Disabled “Host wp-includes/ files” because some of these files were sitting in the upload queue with “Object Already Exists” message

    2.) Enabled “Force over-writing of existing files”

    3.) Disabled “Enable changed files auto-upload”

    4.) Cleared the Minify Cache

    5.) Then selected “Upload Minify Files”

    Back in business.
