Plugin won't let me "Add a Field"

1) Enable Paypal payments checkbox is greyed out ( I can’t check or uncheck it) although it looks checked by default.
2) The form builder section contains only 3 links but no fields. When I click the “add a field” link the page moves down to the next section. No field is created.
I’m using WordPress version 3.2.1
