Plugin won't upload properly

when uploading the plugin zip folder through the wordpress API the folder doesn’t upload or unpack properly. A string of text asking, “Are you sure you want to do this?” displays without any buttons to press…and a link below that string that says, “Please try again.”

any guidance would be appreciated.
