[Plugin: WordPress Forms] Form won't send anything 3 other issues

Hi!

I’ve created a form (which was very easy!) and see 4 problems with it.
Most importantly, it doesn’t email anything when you press ‘submit’.

Then there are 3 smaller issues:
1. Above the area with the check boxes you can see the text is in capital letters, while all the other descriptions aren’t.
2. The gray part of the ‘name box’ is not wide enough
3. When you want to check a box you have to click it twice
