[Plugin: WordPress SEO by Yoast]-Tittle duplicated.Any solution?

    I am first time user of the wordpress seo Yoast. After I activated the plugin, my tittle seems to be duplicated. It become something like [Mindsparker.com-1/2-Mindsparker.com] instead of [Mindsparker.com].
    Any solution to this?

    As for info, I have added the Title and Meta tag manually before using this plugin at the header.php file. Is it advisable for me to remove it?
    Pls kindly help me to scrutinize my source code to find the problem. Mindsparker.com
