[Plugin: WordPress Simple Survey] Intro not being shown, next button never lights up with an answer

  • What could I be doing wrong, everything seems to be installed correctly. The survey shows up, but I never see the intro, and I can never move on to question 2. Survey is located here torringtoneyecare dot com / survey
