[Plugin: WordPress-to-lead for Salesforce CRM] Copy to sender AND lead source (specific)

I suggest, and if it exists please tell me!!!!, that the sender of the message can choose to receive a copy.

Also it is very important for my business (I use the form in several pages and I use the widget in more that 7 places on my site) for the new lead source to be identified by which place within my website he came from: contact us page?, a product page?, a specific post? or any other place I put it on my site. I suggest form the message to pick-up the URL from where it comes from so I can know what the lead did!!!!!
