[Plugin: WP Download Codes] Add new release error

When I attempt to add a new release, I put in the name, select the file and number of allowed downloads. When I click “Submit”, it takes me back to the “Manage Releases” page and tells me “The release was added successfully” but the no releases appear on the list and when I go to “Manage Download Codes” it tells me “No release has been created yet.” I’ve tried different browsers, as well as different combinations of titles and file names, but it still doesn’t work.
