I have already tried enabling “Disable HTTP Caching” without any change.

I also have this issue. I’m using version 0.2.9.19.

It appears to be caused by a line in classes/File.php in the ‘CreateThumbnail’ function (line number 240) where it gets the wrong extension for the thumbnail.

It is using the main file extension instead of the thumbnail extension and so you end up with file names like ‘thumb_mythumbnail.png200x200.pdf’.

Regarding my comment above and here ( https://fabi.me/wordpress-plugins/wp-filebase-file-download-manager/comment-page-39/#comment-12989 ) about Safari not displaying thumbnails.

Turns out Safari doesn’t like the thumbnail files having the PDF extension or it somehow is blocking them.

So instead, I name the pdf whatever.pdf and the thumbnail whatever.jpg and drop them directly on the server and then do a sync within the plugin and the thumbnails display just fine in Safari.
