[Plugin: WP Survey And Quiz Tool] Question with answer matrix

I was wondering if it’s possible to add a question with a answer matrix like the following:

I would like to know if this is easy to do and maybe someone could point me in the right direction to do it.

Perhaps there are other plugins who provide this functionality? I need it for a project i’m currently working on so speed is an issue…
