[Plugin: YIKES, Inc. Easy MailChimp Extender] When clicking on submit nothing happens

Hey,

thanks for your great plugin.

I’m using your plugin in combination with a div container on my sidebar and so far everything works well.

But when I copy the code and e.g. put it on a page or in another sidebar text widget the form does not work anymore. When clicking on submit the page then just reloads.

What yould it be? Please help.
